SOCIAL WORKER

We require the services of a Social Worker to work half day. The successful candidate will work collaboratively with St Luke’s Combined Hospice’s Inter-Disciplinary Team (IDT) to provide psychosocial support to client patients, their family members and significant others.

ENTRY LEVEL QUALIFICATION, KNOWLEDGE AND EXPERIENCE

  • Bachelor of Social Science in Social Work
  • A minimum of 3 year’s related experience
  • Current registration with SACSSP
  • Unendorsed driver’s licence
  • Must be computer literate
  • Speak a minimum of two of the official languages of the Western Cape

KEY PERFORMANCE AREAS

  • Provide holistic psychosocial assessment of patients
  • Provide counselling support to patients and families
  • Facilitate material support of food parcel distribution and grant applications
  • Perform crisis interventions when necessary
  • Compile and submit reports e.g. Psycho Social Assessments and Client Statistics
  • Attend meetings and workshops
  • Perform other duties, as and when so requested, in support of team goals
